1. Register on SchoolMint

• Log onto creoprep.schoolmint.net

• Click Register

• Complete all forms

2. Submit documents to Creo by July 31!

Please email clear and legible photos of documents to enroll@creoprep.org or review Step #3 if you would like to bring them into the school.  If you have any questions, please contact the school at 347-216-9246.

• Student Birth Certificate

  • Passport is acceptable if birth certificate is unavailable or in the process of being requested

• Two forms of Proof of Address

  • Examples of accepted documents include:

    ID, Con Edison Bill, Utility Bill, Lease, Pay Stub, Tax Document, Letter from the city agency

  • Document must show guardian’s full name & address 

  • If you do not have any of these documents under your name, please contact the school for an affidavit of residency form

• Student Medical Physical

  • This must be dated within the past year. If you are waiting for an appointment please contact the school with the data of the upcoming appointment. 

• Student Immunization Record

  • This must be up to date. If you are waiting for an appointment please contact the school with the data of the upcoming appointment. 

• Other

  • If your child has asthma and has an inhaler please send us an Asthma/Inhaler Administration Form

  • If your child has any medical condition (allergy, etc.) please send us a Medication Administration Form.

  • If your child has an Individualized Education Plan (IEP) please send us a copy of the IEP, which you can request from your child’s current school. 

  • If there is a person/persons who are not authorized to pick up your child due to a court/custody order please send us a copy of the Court Order.

3. Uniforms
All new incoming students receive TWO sets of uniforms for FREE during their first year! Each uniform set includes:

• Creo Short Sleeve Polo Shirt

• Creo Long Sleeve Crewneck Sweater

• Khaki Pants

Students will also receive ONE pair of Creo Logo Joggers
In addition to the provided uniform, students must wear 100% Black Sneakers. Parents and students are responsible for the purchase and use of required sneakers.
